Our methodology follows a 35-30-35 Care Coordination Model: 35% patient journey mapping, 30% workflow and system connection, and 35% service visibility and operational improvement.

We start by mapping the patient journey from intake to referral, scheduling, and follow-up. Then we connect the workflows between teams and systems. Finally, we add visibility, tracking, and improvement loops so patients do not fall through administrative gaps.

A referral is sent, but without clear ownership it can sit in an inbox, miss scheduling, or disappear before the patient receives care. The stack tracks each referral from request and triage through booking and appointment completion, giving teams timely prompts to intervene and reducing the chance that patients fall out of the journey.
